You may request a Duplicate Diploma or Education Verification from the Tennessee Department of Education.

A duplicate diploma can be requested by downloading and completing the Duplicate Diploma Request Form. This is a PDF that must be printed and mailed to the Tennessee Department of Education at the address provided on the form. The charge for a duplicate diploma is $10.00 each and must be submitted as a money order along with the form to TDOE.
Tennessee public high school verification can be requested by completing the Verification of High School Completion Form. This is a PDF that must be emailed to [email protected].
